
Referral / Enrollment
Thank you for providing Project Calm, LLC with the opportunity to support you through our array of holistic therapeutic services. It is our desire to ensure the enrollment in services is streamlined and simplified to fit your needs. Thus, we have provided an outline below to give you an overview of what to expect, as well as a questionnaire to complete to help us determine the type of support services you may need. We have also listed the items you should bring to your initial appointment. Again, thank you for allow us to assist you and we look forward to working with you.
If you are seeking to refer someone for treatment, please complete the referral form provided and fax it to our office. Thank you for thinking of Project Calm, LLC and we look forward to collaborating with you.
Intake Procedures
During the initial visit, we will complete intake paperwork outlining past treatment history, presenting problems, listing emergency contacts and executing consent forms. We will complete a clinical assessment to determine the appropriate course of treatment. The Clinical Assessment is a tool used to help document how long the symptoms / concerns have been occurring. It is also used to collect information on the family's history, and to determine what diagnosis the individual may have. Once the assessment is complete, it is time to prepare the treatment plan.

Once we complete the assessment, we will meet to review the findings and, together, decide what goals to work on. During this time, a schedule should be set for days treatment will be delivered.

How Do You Pay For Treatment?

During the initial intake, your insurance information will be collected, along with a copy of your insurance card. The billing department will attempt to receive payment from your carrier first. If there is a problem with receiving payment from your carrier, we will contact you to discuss payment arrangements.
What You Should Bring to Your Initial Appointment
Once your initial appointment is scheduled, you should bring the following items to the first session:

Bring a state identification or Driver's License

Bring your insurance card.

If a minor child is receiving services, bring a copy of the birth certificate.

If you are not the minor child's biological parent, you must provide documentation that you are the legal guardian.
